Keep the individual descriptions and their uncertainty.
Read the five original observations in context, with category records and free support information. No clinical score or drinking advice.
For each original statement, choose whether it describes your past four weeks, partly describes them, or does not describe them. Cannot judge, no relevant experience and prefer not to answer remain three separate categories. These labels have no numerical values. All twenty selections finish the record. Each theme retains five individual selections and six category counts adding to five. Counts are not symptom counts, severity, frequency, a drinking allowance or a comparison between themes. No total, average, cutoff, risk band or personal type is calculated.
A sensation is a report, not an explanation
These prompts concern perceived changes in alcohol’s effect, feeling physically unwell when a usual drink did not happen, drinking to ease a physical feeling and uncertainty about the relation between drinking and the body. They record what you recall noticing. They cannot establish tolerance, dependence, withdrawal, a cause of symptoms or any medical condition.
The two effect-change statements may overlap
Needing more for a similar feeling and noticing less effect from the same amount can describe overlapping impressions. They are retained separately to show what was endorsed, not counted as independent medical symptoms. The last prompt concerns uncertainty and must not be treated as a severity item or reversed into certainty.
No do-it-yourself dependence test
No answer asks you to delay a usual drink, stop suddenly, drink more, recreate discomfort or check whether another drink changes a symptom. Such actions are not part of this exercise. Alcohol withdrawal can require medical care, and this form cannot determine whether it is present or how to manage it.
Unknown does not mean medically clear
You may be unable to judge a bodily change or prefer not to describe it. Those categories preserve uncertainty rather than support a normal result. No fitting answers also cannot exclude a medical issue. You can seek help for an actual concern without answering the other themes or waiting for a pattern to repeat.
An individual conversation can use different evidence
A health professional may ask about timing, actual symptoms, medicines and medical history in an appropriate private setting. This website does not collect those details or substitute its categories for that process. You decide whether to mention a concern; there is no need to share the questionnaire, publish a record or supply a narrative here.
How the record works
For each original statement, choose whether it describes your past four weeks, partly describes them, or does not describe them. Cannot judge, no relevant experience and prefer not to answer remain three separate categories. These labels have no numerical values. All twenty selections finish the record. Each theme retains five individual selections and six category counts adding to five. Counts are not symptom counts, severity, frequency, a drinking allowance or a comparison between themes. No total, average, cutoff, risk band or personal type is calculated.
Origin, limits and support
- FermatMind authored these twenty statements and four complete readings. This is an adult self-description exercise, not AUDIT, AUDIT-C, a diagnostic checklist or a validated translation of another instrument. Reliability, validity, norms and English–Chinese equivalence have not been established. It does not assess dependence, intoxication, withdrawal, physical health or whether a change in drinking would be safe. Public health information supplies context, not validation of this record.
- Support does not depend on any answer or count. If you are concerned about drinking or bodily symptoms, a qualified health professional can assess your actual circumstances. This exercise gives no instructions to stop suddenly, reduce, continue drinking or manage withdrawal. Alcohol withdrawal can be dangerous; seek medical advice before a sudden change if dependence or withdrawal may be involved. For an immediate danger or severe current symptoms, use local emergency medical care instead of this exercise.
