Keep the individual descriptions and their uncertainty.
Read the five original observations in context, with category records and free support information. No clinical score or drinking advice.
For each original statement, choose whether it describes your past four weeks, partly describes them, or does not describe them. Cannot judge, no relevant experience and prefer not to answer remain three separate categories. These labels have no numerical values. All twenty selections finish the record. Each theme retains five individual selections and six category counts adding to five. Counts are not symptom counts, severity, frequency, a drinking allowance or a comparison between themes. No total, average, cutoff, risk band or personal type is calculated.
Effects are specific to an activity
These five prompts concern postponing a responsibility, next-day interference, changes to shared plans, disagreement about an effect and setting aside a valued activity. They retain your account without deciding fault, family roles or what someone else experienced. A reported effect is not a complete history, and a category count cannot show its seriousness or cause.
The same count can describe different days
Changing one shared plan is not identical to repeated difficulty with a responsibility. Both can fit different sentences, yet neither the number of fitting statements nor the number of missing statements establishes how often something occurred. The form does not observe work, relationships or daily functioning and assigns no overall impairment result.
No moral or relationship verdict
A description of drinking-related impact does not define a person as irresponsible. Equally, non-endorsement does not prove that every responsibility or relationship is unaffected. Do not use the record to rate another person, settle a disagreement or decide whether someone is safe to trust. It is a voluntary account by the person answering.
Keep life effects separate from bodily sensations
Daily effects may be described even when bodily changes are unknown, and bodily concerns may matter even when no responsibility was postponed. One theme is not a prerequisite for another. The four records stay side by side without compensation, ranking or a combined conclusion about health.
You choose what, if anything, to discuss
You may keep a recollection private or choose to discuss an actual concern with a qualified professional. There is no requirement to contact someone involved, seek confirmation, disclose a conflict or perform a relationship exercise. Seeking help does not require a fitting answer, a completed form or a particular number of effects.

Origin, limits and support
- FermatMind authored these twenty statements and four complete readings. This is an adult self-description exercise, not AUDIT, AUDIT-C, a diagnostic checklist or a validated translation of another instrument. Reliability, validity, norms and English–Chinese equivalence have not been established. It does not assess dependence, intoxication, withdrawal, physical health or whether a change in drinking would be safe. Public health information supplies context, not validation of this record.
- Support does not depend on any answer or count. If you are concerned about drinking or bodily symptoms, a qualified health professional can assess your actual circumstances. This exercise gives no instructions to stop suddenly, reduce, continue drinking or manage withdrawal. Alcohol withdrawal can be dangerous; seek medical advice before a sudden change if dependence or withdrawal may be involved. For an immediate danger or severe current symptoms, use local emergency medical care instead of this exercise.
