Keep the individual descriptions and their uncertainty.
Read the five original observations in context, with category records and free support information. No clinical score or drinking advice.
For each original statement, choose whether it describes your past four weeks, partly describes them, or does not describe them. Cannot judge, no relevant experience and prefer not to answer remain three separate categories. These labels have no numerical values. All twenty selections finish the record. Each theme retains five individual selections and six category counts adding to five. Counts are not symptom counts, severity, frequency, a drinking allowance or a comparison between themes. No total, average, cutoff, risk band or personal type is calculated.
Context is not a measured risk level
This theme records alertness-related responsibilities, uncertainty about medicines, a noticed safety concern, continuing after an unwanted effect and difficulty leaving a drinking setting. It does not calculate intoxication, a legal limit, driving fitness, medicine compatibility or likelihood of harm. Any actual safety question requires information this questionnaire does not collect.
A question can remain unanswered numerically
Not knowing whether alcohol and a medicine are compatible is different from having no relevant medicine experience. Prefer not to answer is different again. Each receives its own category, rather than being converted into no risk or a midpoint. The record supplies no list of compatible medicines and no permission to try a combination.
One occasion cannot be reconstructed here
A selected statement does not establish who was present, whether pressure was involved, what transport was available or how an unwanted effect developed. The record cannot determine consent, legal responsibility or another person’s intentions. No count can make an actual unsafe situation safe.
No comparison that cancels an actual concern
A person may endorse one context and none of the other four. That pattern neither lowers the importance of the actual situation nor establishes a general risk trait. This page does not compare people or assign a low-risk group. A public explanation remains available even if every prompt is withheld.
No reenactment or safety experiment
You do not need to revisit a venue, try drinking with a medicine, test alertness or ask someone to recreate an event. For a real concern, use appropriate medical or local safety support rather than the questionnaire. This reading offers context for the words, not instructions for managing a hazardous situation.
How the record works

Origin, limits and support
- FermatMind authored these twenty statements and four complete readings. This is an adult self-description exercise, not AUDIT, AUDIT-C, a diagnostic checklist or a validated translation of another instrument. Reliability, validity, norms and English–Chinese equivalence have not been established. It does not assess dependence, intoxication, withdrawal, physical health or whether a change in drinking would be safe. Public health information supplies context, not validation of this record.
- Support does not depend on any answer or count. If you are concerned about drinking or bodily symptoms, a qualified health professional can assess your actual circumstances. This exercise gives no instructions to stop suddenly, reduce, continue drinking or manage withdrawal. Alcohol withdrawal can be dangerous; seek medical advice before a sudden change if dependence or withdrawal may be involved. For an immediate danger or severe current symptoms, use local emergency medical care instead of this exercise.
