Keep the individual descriptions and their uncertainty.
Read the five original observations in context, with category records and free support information. No clinical score or drinking advice.
For each original statement, choose whether it describes your past four weeks, partly describes them, or does not describe them. Cannot judge, no relevant experience and prefer not to answer remain three separate categories. These labels have no numerical values. All twenty selections finish the record. Each theme retains five individual selections and six category counts adding to five. Counts are not symptom counts, severity, frequency, a drinking allowance or a comparison between themes. No total, average, cutoff, risk band or personal type is calculated.
Intention and what followed
This theme separates an intended amount or duration from what you recall doing. It also records attention returning to drinking, difficulty keeping a chosen limit and time used arranging a drink. Endorsing a sentence describes only that sentence within four weeks. It does not show how often the event occurred, how much was consumed or why it happened. None of the five prompts is a test of willpower or character.
Two plans can involve different circumstances
One person may recall a planned end time changing while not endorsing the statement about repeated thoughts. Another may think about drinking without drinking beyond a plan. The same category counts can conceal these differences, so the individual selections stay visible. The record cannot reconstruct an occasion or decide whether a remembered plan was realistic.
A plan is not a safe-drinking limit
A chosen limit is your recollection, not a limit this website endorses. Keeping to it does not establish safety, and exceeding it does not by itself establish a disorder. This form collects no dose, medical history, medicine details or observation from another person. It cannot calculate safe consumption or substitute for an individual health assessment.
Read alongside effects without adding them
A mismatch with a plan and an effect on responsibilities are different descriptions. Neither one explains or cancels the other. You do not need to find a dominant theme, compare yourself with someone else or repeat the form to prove improvement. A partly fitting answer can remain partly fitting without being forced into yes or no.
An optional private recollection
If useful, notice which part of one statement you recognised and which part remains uncertain. No diary, name, amount, explanation or disclosure is required. You do not need to recreate a drinking situation, test a limit or change consumption to complete this reflection. Reading may be the end of the activity.
How the record works

Cannot judge, no relevant experience and prefer not to answer remain three separate categories. These labels have no numerical values. All twenty selections finish the record. Each theme retains five individual selections and six category counts adding to five. Counts are not symptom counts, severity, frequency, a drinking allowance or a comparison between themes. Origin, limits and support
- FermatMind authored these twenty statements and four complete readings. This is an adult self-description exercise, not AUDIT, AUDIT-C, a diagnostic checklist or a validated translation of another instrument. Reliability, validity, norms and English–Chinese equivalence have not been established. It does not assess dependence, intoxication, withdrawal, physical health or whether a change in drinking would be safe. Public health information supplies context, not validation of this record.
- Support does not depend on any answer or count. If you are concerned about drinking or bodily symptoms, a qualified health professional can assess your actual circumstances. This exercise gives no instructions to stop suddenly, reduce, continue drinking or manage withdrawal. Alcohol withdrawal can be dangerous; seek medical advice before a sudden change if dependence or withdrawal may be involved. For an immediate danger or severe current symptoms, use local emergency medical care instead of this exercise.
